How do I prevent this interview with terrifying me?

Dear Harriette: I have been working as an associated product manager for two years, and I am ready to take the next step in my career interviewing for a new position. However, I am terrified because I have always considered myself a terrible interviewee.

I have trouble answering questions at the spot, I feel that it seems nervous and without preparation, and blank when it is time to talk about my achievements. The idea of facing another round of interviews makes me so anxious that I have been postponing the job application completely.
